● At the top right corner of the page, there is a red bar that says “Log in/Register”. Click on that link.

●  On that next page, click the Blue bar that says “sign up for free”.

●  Fill in the appropriate information on the next page and click submit.

● You will be sent a verification code, either to the email address of the phone number you listed. It will be sent immediately. Use that code to finish the initial sign up.

● You will then be taken to a page that asks you to identify yourself as parent, coach, admin, etc. Choose parent.

● Next you will need to type in our team code. It is DF- Q3MAQK Hit Next.

● At this point, you will be directed to the forms that need to be filled out. You will add your athlete/athletes and fill out the necessary paperwork for that athlete. There are 6 forms that need to be filled out and each one has one or more electronic signatures on it. They are all the forms required by the county for every athlete on the team. These forms will stay with the athlete and go to every team they play for at Stewarts Creek Middle School. If your player has already completed an account and completed the appropriate pages for another sport, then there is no need to do it again. 

● When you log in, there is a button below your name that says 2023-24 Preparticipation Forms. All forms must be completed.

● From here on out, all you will ever have to turn in to the coach is the physical signed by the doctor. We will add that to the athlete’s account.
